THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
This Notice describes how Serenity Human Service Coordination, LLC (“Serenity”) may use and disclose health information about you, your rights regarding that information, and our responsibilities for protecting it.
Your Rights
You have certain rights regarding your health information. This section explains those rights and some of our responsibilities in helping you exercise them.
Get an Electronic or Paper Copy of Your Health Information
You may ask to inspect or obtain an electronic or paper copy of health information that we maintain about you.
We will provide access or a copy as required by applicable law. We may charge a reasonable, cost-based fee when permitted by law.
In limited circumstances, we may deny access to certain information when permitted by law. When applicable, we will explain your rights regarding that decision.
Ask Us to Correct Your Health Information
If you believe health information we maintain about you is incorrect or incomplete, you may ask us to correct or amend it.
We may deny your request in certain circumstances permitted by law. If we deny the request, we will provide an explanation when required.
Request Confidential Communications
You may ask us to communicate with you about your health information in a particular way or at a particular location.
For example, you may ask us to contact you only at a particular telephone number or to send correspondence to a particular address.
We will accommodate reasonable requests as required by law.
Ask Us to Limit What We Use or Disclose
You may ask us not to use or disclose certain health information for treatment, payment, or healthcare operations.
We are generally not required to agree to every requested restriction.
If you pay for a healthcare service or item completely out of pocket, you may ask us not to disclose information about that service or item to your health plan for payment or healthcare operations. We will honor the request when required by law unless the disclosure is required by law.
Receive an Accounting of Disclosures
You may request a list of certain disclosures we have made of your health information.
The accounting generally covers certain disclosures made during the six years before your request and does not include every type of disclosure.
We will provide an accounting as required by applicable law.
Get a Copy of This Notice
You may request a paper copy of this Notice at any time, even if you previously agreed to receive the Notice electronically.
Choose Someone to Act for You
If you have given someone medical power of attorney, or another person has legal authority to act on your behalf, that person may exercise your rights and make choices about your health information as permitted by law.
We may verify the person's authority before taking action.
File a Privacy Complaint
You may file a complaint with Serenity if you believe your privacy rights have been violated.
You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
Serenity will not retaliate against you for filing a privacy complaint.
Your Choices
For certain health information, you may tell us your preferences about what we disclose.
Depending on the circumstances and applicable law, this may include sharing information with family members, friends, or other people involved in your care.
If you are unable to tell us your preference, such as when you are unconscious or otherwise unable to communicate, we may disclose information when permitted by law and when we determine that doing so is in your best interest.
How We May Use and Disclose Your Health Information
HIPAA and other applicable laws allow or require us to use and disclose health information in certain circumstances. Examples are described below.

We may use and disclose your health information to provide, coordinate, or manage your healthcare.
For example, we may share relevant information with another healthcare professional involved in your treatment or care coordination when permitted by law.

Healthcare operations may include activities such as quality assessment, staff training, supervision, compliance, auditing, case management, credentialing, licensing activities, and other operations permitted by law.

Mental and Behavioral Health Information
Because Serenity provides mental and behavioral health services, some information we maintain may be subject to privacy protections that are more restrictive than the general HIPAA requirements.
When federal or Idaho law provides greater protection for particular health information, Serenity will follow those requirements.
Certain uses and disclosures of psychotherapy notes require your written authorization except when the use or disclosure is otherwise permitted or required by law.
Other Uses of Your Health Information
Uses and disclosures of your health information that are not otherwise permitted or required by law generally require your written authorization.
If you give us written authorization, you may revoke that authorization in writing as permitted by law.
Revoking an authorization will not affect actions we already took in reliance on your authorization before receiving the revocation.
Our Responsibilities

We are required to provide you with this Notice describing our legal duties and privacy practices regarding protected health information.
We must follow the duties and privacy practices described in the Notice that is currently in effect.
